Options for Community Living, Inc. is looking for a Quality Improvement (QI) Specialist to join our team! An ideal candidate should meet the following requirements:

  • High School Diploma required. Bachelor's degree in health and human services, social work or related field preferred.
  • At least 2 years of experience working in a human services related field.
  • Experience with computer software programs including Excel, Access, Word and/or other database applications preferred.
  • Experience using Foothold (Awards) and/or Foothold Care Management electronic medical record systems, with a strong understanding of reporting preferred.
  • Must be available to travel to sites throughout Nassau and Suffolk Counties, including, but not limited to, the Hempstead and Amityville offices and the community residences.
  • Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle is required.

Physical Demands: Must be able to sit and look at a computer screen for long periods of time. Must be able to speak for periods of 15-30 minutes.

Salary Range:

$43,680/yr. ($24.00/hr.) – $54,500/yr. ($30.00/hr.)

Salary offers will be commensurate with experience and other qualifications.

Schedule:

Monday - Friday: 8:00 AM - 4:00 PM, 8:30 AM - 4:30 PM or 9:00 AM - 5:00 PM (35 hours/week)

Location: In-person based out of our Ronkonkoma office.

Pay Type: Non-exempt

Responsibilities:

The Quality Improvement Specialist is responsible for assisting the Quality Improvement department with various audit and compliance activities to ensure compliance with regulations and agency policies and procedures.

  • Lead the Agency’s record review process; Compile program wide record review logs; Select records to be reviewed; Complete a sample of record reviews.
  • Conduct regular record audits ensuring compliance with Options’ Policies, OMH, DOH, Health Home, Medicaid and other contracts as applicable.
  • Complete compliance checks including but not limited to: ATC Care Coordination Note Quality Check, MH Licensed Service Plan Linked Note Quality Check, RWB Health Education Quality Checks, etc. Report to program management weekly.
  • Conduct regularly scheduled medication audits; Provide guidance and training related to issues found; provide input regarding policies and procedures related to medication.
  • Complete data-entry from outcome forms, safety reports, discharge follow-up forms, and other forms as needed.
  • Conduct Quality of Service calls following contact conducted with clients in the ATC Health Home Care Coordination Program, ATC Housing, ATC Ryan White, Mental Health Supportive Housing Program, MH Licensed Programs, HCBS and MRST by telephone. Document all results and notify management of any serious issues immediately.
  • Identify issues found from the safety reports along with areas for improvement and communicate to applicable staff. Run necessary reports; Follow-up with staff as necessary.
  • Set up new users in Foothold Electronic Health Record; Maintain users and permissions when staff transfer or exit the Agency.
  • Support database functions as needed.
  • Assist in administering client satisfaction surveys annually.
  • Serve as Foothold Super-user and assist with responding to and correcting issues sent to the Foothold helpdesk.
  • Assist Corporate Compliance Officer and Compliance and Quality Coordinator in carrying out investigations, including documentation review, conducting interviews, compiling documentation package, etc.; may lead investigations as needed.
  • Other relevant tasks as assigned by supervisor.
